Below you can download and view our consent forms for this study. If you are interested in participating, please email us at yale.cdd@yale.edu or fill out the contact us form and we will provide you a link to sign the consent forms.
For each family participating, we will need a general compound consent for each parent or adult sibling to sign and represent themselves. We will also need a general parental permission consent for any affected individual as well as any unaffected siblings under the age of 18 signed by the parents. For children between ages 7-17 who can understand they will be participating, we will need an assent form signed by them to acknowledge they are willing to participate. Any questions about the consent forms, please contact us and we can review the documents with you. We also ask for a health information release form for the affected individuals so we can contact their physicians.
